Your login username is your email address that we have on file. If you enter your email address in the password reminder field, your password will be emailed to you. Go to the login page by clicking the "Practitioners Logon" link at the bottom of any page, click the "click here if you forgot your password" link, and then enter your email address. If for any reason this does not work, either the email address you are using is not the one for your listing or we are having problems sending you and email, please email us and let us know that the password reminder form did not work for you. |
